Step 1: Fill out the enrollment questionnaire e-mailed to you. This survey will ask questions about the type of pain you have, the medications you are taking, and the the treatments you have tried. You will receive a $10 Amazon gift card for completing this step.

Step 2: Before each of your doctor visits, we will e-mail you some materials to help you prepare for the appointment. Print out the materials, fill out the “Preparing for Your Visit” sheet and bring it to your doctor.

Step 3: After a visit with your doctor, we will send you another survey to ask about your appointment. You can fill this survey out on your phone or computer.

Step 4: Every month, we will check in with you by sending another survey asking about your overall health.
